Change of circumstances form

This page explains when you should use the change of circumstances form for the points-based system, the information you should include, and where you should send the form.

When to use the change of circumstances form

You should use this form to notify us of any changes to your details while you are in the UK, before or after your application has been decided.

You should use this form to notify us of any changes to your details if you are in UK. This includes:

  • changes to an application before it has been decided;
  • changes to an application after it has been decided; 
  • reporting details of your criminal convictions (if any);
  • the type of change, for example, to your contact details or the details for your representative or dependents.

You must fill in the form in block capitals using a black pen. Please enter all dates as dd-mm-yyyy, for example 29-04-2000.

Reporting changes for biometric residence permit (BRP) holders

If you hold a current biometric residence permit (BRP) and want to tell us about changes to your:  

  • name;
  • date of birth;
  • nationality;
  • gender; or
  • appearance.

You must apply for a new BRP and report the changes using the application form BRP (RC). You should also use this form if you need to replace a BRP that has been lost, damaged or stolen. The form can be downloaded from the Biometric residence permit holders' responsibilities  page.

What information do you need to include?

When you complete the form, you should include the following information:

  • a description of the change;
  • your reference number for the points-based system; 
  • your representative's reference number (if appropriate); 
  • your date of birth; and
  • your nationality.

We will write to you to confirm that we have noted your change of circumstances. You should keep this letter with the original documents from your application.
