Change of circumstances form

This page explains if you are inside the UK when you should use the change of circumstances form or the points-based system, what information you need to complete it and where you should send it.

When to use the change of circumstances form

You should use this form to notify us of any changes to your details if you are in UK. This includes:

  • changes to an application before it has been decided;
  • changes to an application after it has been decided; 
  • reporting details of your criminal convictions (if any);
  • the type of change, for example, to your contact details or the details for your representative or dependents.

You must fill in the form in block capitals using a black pen. Please enter all dates as dd-mm-yyyy, for example 29-04-2000.

Reporting changes for biometric residence permit (BRP) holders

If you hold a current biometric residence permit (BRP) and want to tell us about changes to your:  

  • name;
  • date of birth;
  • nationality;
  • gender; or
  • appearance.

You must apply for a new BRP and report the changes using the application form BRP (RC). You should also use this form if you need to replace a BRP that has been lost, damaged or stolen. The form can be downloaded from the Biometric residence permit holders' responsibilities page.

What information you need to complete the change of circumstances form

You will need the following information to complete the form:

  • a description of the change you want to make; 
  • your PBS reference number; 
  • your representative reference number; 
  • your date of birth; 
  • your nationality.

We will write to you to confirm your change of circumstances form has been received. You should keep this letter with the original documents from your application.
