What to send with your application form
This page explains what you should send us with your application form for a Home Office travel document.
You must send us:
- the appropriate fee for the type of document you are applying for;
- two photographs, as detailed below;
- proof of your change of name, if this applies to you, such as your marriage certificate;
- the evidence we require to support your application, as detailed on the pages for individual types of travel document;
- the original letter from the Home Office or UK Border Agency that confirms your status as a refugee or stateless person, or other status in this country, and shows that you have permission to stay here;
- the original (not a copy) of your United Kingdom residence permit, if you have one;
- any passport or travel document that you have, even if it is no longer current or valid (this must be the original, not a copy);
- any further information that we require for particular types of travel document or for particular circumstances (such as to prove an urgent need to travel).
You must send your original Home Office travel document, declaration of identity for visa purposes (known as a GV3), or the European Union identity document known as a uniform format form (UFF). If your passport or Home Office travel document has been lost or stolen, you must send a full police report of this with your application, together with a full explanation of how it was lost or stolen.
Photographs
You should send us two identical, recent, passport-size photographs of yourself and of each of your dependants applying on the same form. Write the name of the person on the back of each photograph.
All the photographs should:
- measure 45mm by 35mm;
- be in colour;
- be taken against a white background;
- be clear and of good quality;
- not be framed or backed; and
- show your full face, without sunglasses, hat or head covering unless you wear this for religious reasons.
You should not pin, staple, glue or tape the photographs to your application form.
Applications by other members of your family
If other members of your family are applying for Home Office travel documents, please send us all the applications together in one envelope. However, you must make a separate payment for each application form. See Cost of applying for more information about payment.
