Completing the application form
This page explains how you should complete the declaration to give up your British citizenship or other British status.
Applications should be made on application form RN.
The application form is split into two sections. The first two pages of the application form are the official copy. This section of the application form will be kept by us once you have made your application. The third page of the application form is the applicant's copy. This section of the application form will be signed, stamped and returned to you if your declaration is registered.
Parts 1- 6
You should complete parts 1 - 6 of the official copy and parts 1 - 5 of the applicant's copy. It is important that the details you give on parts 1 - 4 are exactly the same as on both copies of the form.
You must sign and date part 6 of the official copy and part 5 of the applicant's copy. If you do not sign and date both these sections your declaration will not be valid.
Part 7 - the countersignatory
The countersignatory is another person who must also sign your form, to confirm that you are of sound mind. This part must be completed by someone who is aged 18 or over and who knows you personally.
