Completing the application form

This page provides details of how to fill in the registration application form RS2. Before you fill in the application form you should read the section on eligibility to make sure you meet the requirements. If your application is refused because you do not meet the requirements your fee will not be fully refunded.

You must fill in the form in block capitals using a black or dark-blue pen. Please enter all dates as dd-mm-yyyy, for example 29-04-2000.

You need to complete all sections of the application form and sign and date the declaration at the end. If you do not sign and date the declaration, your form will not be valid.

The form has not yet been updated to reflect changes in the rules, which now allow both men and women to apply for registration through connections by marriage or civil partnership to a citizen of the United Kingdom and Colonies.

If necessary, when completing part 11, cross out 'husband' and write 'wife' or 'civil partner' and give us your wife or partner's details (and, if appropriate, those of his/her father or father's father).

You should also include your husband's, wife's or civil partner's father's details if they are appropriate. These details have not been requested on the application form, you should add them at the end of part 11 if necessary.
