Completing the application form

This page provides details of how to fill in the registration application form MN2. Before you fill in the application form you should read the eligibility section to make sure you meet the requirements. If your application is rejected because you do not meet the requirements, your fee will not be refunded.

You must fill in the form in block capitals using a black or dark-blue pen. Please enter all dates as dd-mm-yyyy, for example 29-04-2000.

The application should be made by a parent or guardian of the child.

If the child meets the requirements in section C of the eligibility information, both parents must agree to the application unless they are not married.

If it is not possible for both parents to give their consent to the registration on the application form, written consent can be given in a letter to the governor.

If a parent has not given consent, the reason should be given in part 16.

If a parent objects to registration they should send a separate letter giving reasons to the governor.

The parts of the application form that must be completed will depend on which of the eligibility requirements the child meets.

If the child comes within section A, they should complete parts 1-5, 8, 12-13 and 16 of the application form.

If the child comes within section B, they should complete parts 1-5, 8-9 and 12-16 of the application form.

If the child comes within section C, they should complete parts 1-5, 8-9, 12-14 and 16 of the application form.

If the child comes within section D, they should complete parts 1-13 and 16 of the application form.
